"Reflex to culture" is a lab rule: if the urinalysis shows signs of infection — positive nitrite or leukocyte esterase, or white cells and bacteria on the microscopic exam — the lab automatically ("reflexively") sets up a urine culture on the same sample, then identifies the organism and tests which antibiotics it responds to. The culture is billed as a second test. Ten chemistries on the dipstick — glucose (diabetes), protein (kidney damage), blood, ketones, bilirubin and urobilinogen (liver and red-cell breakdown), nitrite and leukocyte esterase (infection), pH and specific gravity (concentration) — plus a microscopic examination that counts red and white blood cells, epithelial cells, bacteria, casts and crystals. Together they point toward infection, kidney disease, dehydration, uncontrolled diabetes or a kidney stone, and tell your clinician which follow-up test is worth ordering.

No fasting. At the patient service center you will be given a sterile cup: wash your hands, clean the area with the wipe provided, start urinating into the toilet, then catch the middle portion of the stream (a "midstream clean-catch"). A first-morning sample is the most concentrated and best for detecting protein, but any time of day works. Avoid heavy exercise the day before (it can cause transient protein and blood) and tell your clinician about vitamin C or beet intake, which can affect the dipstick.
