The urine albumin-to-creatinine ratio (ACR) detects microalbuminuria — small amounts of the protein albumin leaking through the kidney filters into urine. This is the earliest detectable sign of diabetic kidney disease and hypertensive nephropathy, appearing years before kidney function declines on standard blood tests. The American Diabetes Association recommends annual microalbumin screening for all diabetics. It is also valuable for anyone with hypertension, a family history of kidney disease, or cardiovascular risk factors. Early detection allows for interventions (blood pressure control, ACE inhibitors) that can halt or reverse kidney damage.
