Basic metabolic panel (BMP) = 8 values: glucose, calcium, sodium, potassium, chloride, carbon dioxide (bicarbonate), BUN, creatinine — kidneys, blood sugar, electrolytes, acid–base. Prothrombin time (PT) measures how many seconds plasma takes to clot through the extrinsic and common pathways (factors VII, X, V, II, fibrinogen); the INR standardizes PT across labs and reagents. How to read an antinuclear antibody (ANA) result. Reported as a titer (1:40, 1:80, 1:160, 1:320, 1:640, 1:1280 — the highest dilution still positive) and an immunofluorescence pattern. Low titers are common in health: about 13-20% of healthy adults are positive at 1:40 and ~5% at 1:80; 1:160 (~3%) and above carry more weight; 1:640+ is uncommon in health. Titer does not track disease severity and is not used for monitoring. Patterns hint at targets: homogeneous (anti-dsDNA/histone, lupus, drug-induced lupus), speckled (least specific), nucleolar (scleroderma), centromere (limited scleroderma), peripheral/rim (lupus). A negative ANA makes lupus very unlikely but does not exclude RA, spondyloarthritis or vasculitis. Quantitative (beta) hCG reports the hormone level in mIU/mL: below 5 negative, 5-25 indeterminate (repeat in 48 h), 25+ consistent with pregnancy; detectable about 7-10 days after conception, a few days before a urine test. Discriminatory zone ~1,500-2,000 mIU/mL: a gestational sac should be visible on transvaginal ultrasound; a high hCG with an empty uterus prompts evaluation for ectopic pregnancy. Slow rise, plateau or fall can indicate early loss or ectopic but no pattern is diagnostic on its own; after a loss hCG returns below 5 in 2-6 weeks. Non-pregnancy causes of positive hCG: hCG trigger shots (10-14 days), trophoblastic disease, germ-cell tumours, perimenopausal pituitary hCG (5-15), heterophile-antibody interference. How CDC two-tier Lyme testing works and when to use it. Tier 1 is a sensitive ELISA antibody screen; tier 2 is a specific Western blot confirmation that runs automatically ('reflex') when the screen is positive or equivocal — only screen-positive PLUS blot-positive counts as a positive result. Timing dominates accuracy: antibodies take 2-6 weeks to develop, so testing in the first 2 weeks after a bite often reads falsely negative; 4-6 weeks post-exposure is the reliable window, and an early negative with persistent symptoms warrants a retest at that mark. The bullseye rash (erythema migrans) is the exception — treated on clinical sight without waiting for any blood result. IgM appears early and fades (an IgM-only positive months after exposure is viewed skeptically); IgG appears from ~4-6 weeks and persists for YEARS after successful treatment, so a positive test in someone treated long ago usually reflects immune memory, not reinfection, and serology is never a test of cure.
