Cytomegalovirus (CMV) is a common herpesvirus that most people acquire at some point in life, usually with no symptoms at all. IgM antibodies appear within days to weeks of a new CMV infection and typically fade over the following months, so a positive IgM points toward recent or reactivated infection rather than long-past exposure. This test is most often ordered during pregnancy, around transplant, or to work up an unexplained mononucleosis-like illness when the mono test is negative. One important interpretation caveat: CMV IgM on its own cannot reliably separate a true primary infection from a reactivation or a non-specific cross-reaction, and it can persist for several months after the infection resolves. It is normally read together with CMV IgG, and when the timing of a primary infection genuinely matters, an IgG avidity test is what dates it.
