IgG antibodies to cytomegalovirus (CMV) reflect past infection and lasting immune memory. Because CMV stays in the body for life after a first infection, a positive IgG means exposure at some point in the past and does not indicate current illness. A negative IgG is equally informative: it identifies people who have never encountered CMV and are therefore susceptible to a first (primary) infection, which is the scenario that carries risk in pregnancy and around transplant. CMV IgG is the natural companion to CMV IgM — the two together distinguish never-infected from previously infected from possibly recently infected, which no single antibody can do alone.
