Autoimmune gastritis is a slow immune attack on the stomach's parietal cells — the cells that make acid and intrinsic factor, the protein needed to absorb vitamin B12. Left unrecognized it ends in pernicious anemia and, sometimes, nerve damage that does not fully reverse. This panel reads the condition from four angles in a single draw. The intrinsic factor blocking antibody is the specific test: positive in roughly half to two-thirds of pernicious anemia, and when it is positive with a low B12 the diagnosis is made. Gastrin is the functional marker: when the acid-making cells are gone there is no acid to switch gastrin off, so it climbs several-fold. Vitamin B12 shows whether absorption is already failing, and the CBC with differential shows whether that has produced the large-cell (macrocytic) anemia or a falling blood count. Together they answer the question most people ordering this have — is my low or borderline B12 a diet problem or an absorption problem — and give a gastroenterologist what they need to decide on endoscopy. The parietal cell antibody, the more sensitive but less specific screen, is sold separately (it is a Quest test); ferritin is the natural add-on if iron deficiency is part of the picture, because the same loss of acid impairs iron absorption. Gastrin requires a 12-hour fast and is raised several-fold by proton-pump inhibitors; see the prep notes.

Fast 12 hours before the draw (water is fine) — gastrin rises for hours after a meal. If you take a proton-pump inhibitor (omeprazole, esomeprazole, pantoprazole, lansoprazole), ask your clinician whether to pause it beforehand; PPIs raise gastrin several-fold. If you receive B12 injections, draw before an injection or at least a week after one. Do not stop a prescribed medication on your own.

Autoimmune Gastritis Panel results are typically ready in 3–5 business days after your draw at Quest or Labcorp. TestWell emails you the moment they post, and your results dashboard shows each marker against its reference range with a plain-English explanation.
Yes. The fasting before blood work guide covers how long to fast, what you can drink, and which results eating changes.
Common medications, smoking and nicotine and recent blood donation can change an Autoimmune Gastritis Panel result without anything being wrong. Common medications can lower Vitamin B12 and can raise WBC — check B12 periodically on metformin. Smoking and nicotine can raise RBC and WBC — tell the clinician; a high hematocrit in a smoker is usually smoking, not polycythemia. Recent blood donation can lower RBC and can shift MCV — wait 6–8 weeks after a whole-blood donation for an anemia baseline. Each is explained on our physician-reviewed "Before your test" pages; when one applies, remove it and retest.
Order the Autoimmune Gastritis Panel online from TestWell for $199.99 (plus a $6 per-order lab processing fee) — no doctor's visit, referral or insurance needed. A licensed physician authorizes the order, then you walk into any Quest or Labcorp patient service center near you for the draw (the lab locator on our site lists the closest ones by ZIP code), and results arrive in 3–5 business days.
