The short answer
The specific blood test for pernicious anemia — antibodies that block intrinsic factor, the protein your stomach makes so you can absorb vitamin B12, measured from a standard blood draw. At TestWell it costs $159.99 plus a $6 lab processing fee, needs no fasting, and results post in 3–5 business days. Physician-authorized with no doctor's visit or insurance needed; draw at Quest or Labcorp.

Intrinsic factor is made by the stomach's parietal cells and binds vitamin B12 so the small intestine can absorb it. In pernicious anemia the immune system makes antibodies against intrinsic factor, B12 cannot be absorbed from food or ordinary oral supplements, and the deficiency develops over years: fatigue, a large-cell (macrocytic) anemia, and sometimes numbness, balance problems or memory changes that can become permanent if the deficiency is missed. A positive intrinsic factor blocking antibody is highly specific — in a person with low B12 it confirms pernicious anemia — but it is only found in about half to two-thirds of cases, so a negative result does not rule the condition out; the parietal cell antibody is more sensitive and less specific, and the two are usually read together with a B12 level and a complete blood count. Order this test if your B12 is low or borderline without an obvious dietary cause, if you have an unexplained macrocytic anemia, or if you have another autoimmune condition (autoimmune thyroid disease, type 1 diabetes, vitiligo) and new fatigue or neurological symptoms. A recent B12 injection can produce a false positive on some assays; draw before an injection or a week or more after one. Run at Labcorp (010413) or Quest (568).
