Tryptase is an enzyme released primarily by mast cells. Baseline tryptase levels are used to screen for mastocytosis (an abnormal accumulation of mast cells) and mast cell activation syndrome (MCAS) — conditions that cause flushing, hives, gastrointestinal symptoms, and potentially life-threatening anaphylaxis. A baseline tryptase above 20 ng/mL is highly suggestive of systemic mastocytosis. Elevated levels can also be seen after anaphylactic reactions, severe allergic episodes, and certain blood cancers. This test is valuable for patients with unexplained allergic-type symptoms, recurrent anaphylaxis, or suspected mast cell disorders.
