This orders both cytomegalovirus (CMV) antibody classes together from a single blood draw. The two answer different questions and are far more useful read side by side than alone: IgG shows whether you have ever been infected with CMV, while IgM rises within days to weeks of a new or reactivated infection and fades over the following months. Together they separate never-infected from previously infected from possibly recently infected — a distinction neither antibody can make on its own. This pairing is the standard starting point when CMV status matters, most commonly in pregnancy and around transplant. One caveat worth knowing: a positive IgM cannot by itself distinguish a true primary infection from a reactivation or a non-specific cross-reaction, and it can persist for months, so where the timing of a primary infection is clinically decisive an IgG avidity test is what dates it.
