How much do the most common blood tests cost at Quest, Labcorp and TestWell?

Why does the same blood test have three different prices?

Because the price depends on who is billing, not on the test. A hospital outpatient lab bills from its chargemaster and negotiates down with insurers; a self-pay patient often sees the undiscounted number. The labs' own consumer sites sell a fixed cash price that includes physician oversight and the draw. A lab-ordering network like TestWell buys the same test codes at volume rates and adds one flat fee — which is why the identical draw at the identical Quest or Labcorp location can carry three prices.
