Perimenopause is diagnosed by the pattern of symptoms and cycle changes, but blood tests answer the two questions that pattern leaves open. FSH and LH rise as the ovaries respond less to them, and estradiol becomes erratic and then falls; a high FSH with a low estradiol on a day 2–4 draw supports the transition, while a normal result in a woman with irregular cycles does not rule it out — levels swing from month to month in perimenopause, which is why a repeat draw is sometimes more useful than a bigger first one. TSH is included because an underactive or overactive thyroid produces the same fatigue, weight change, sleep disruption, mood change and cycle irregularity, is common in the same decade, and is treatable. This is the lean version of our Menopause & Perimenopause Test Panel, which adds progesterone, testosterone, SHBG and vitamin D for a fuller picture; start here if the question is simply whether the transition has begun.
Hormone testing measures the chemical messengers that regulate energy, mood, sex drive, weight, and reproduction — helping pinpoint imbalances that cause a wide range of symptoms.
Anyone with unexplained fatigue, mood changes, libido changes, weight changes, irregular cycles, or considering hormone replacement therapy.

No fasting required. If you still have periods, draw on day 2–4 of your cycle (day 1 is the first day of full flow) — FSH, LH and estradiol are read against that window. If periods are irregular or absent, any day is fine; a morning draw suits TSH. Stop high-dose biotin for two days beforehand.

Cycle day, biotin supplements and testosterone therapy (TRT) can change a Perimenopause Test result without anything being wrong. Cycle day can shift FSH, LH and Estradiol — draw on cycle days 2–5. Biotin supplements can falsely lower TSH and can shift Estradiol — stop biotin ≥72 hours before the draw. Testosterone therapy (TRT) can raise Estradiol and can lower FSH — measure at the same point in the cycle as testosterone. Each is explained on our physician-reviewed "Before your test" pages; when one applies, remove it and retest.
