What does a low Ferritin mean?

Short answer

Low ferritin means depleted iron stores and is the earliest, most specific marker of iron deficiency — it falls before hemoglobin does. In menstruating women, heavy periods are the usual cause; in men and post-menopausal women, blood loss from the gut needs to be considered even when there are no symptoms. Diet, malabsorption (celiac disease) and frequent blood donation are other common causes.
