The urea breath test is considered the gold standard non-invasive method for detecting active Helicobacter pylori infection. You swallow a small capsule containing urea labeled with a carbon isotope, then breathe into a collection bag. If H. pylori is present in your stomach, it breaks down the urea and releases labeled carbon dioxide, which is detected in your breath.
