The End-of-Year Blood Work Checklist: What to Order in December, in What Order, So the Draw Lands Before December 31 and the Results Land Before Your January Physical
Three reasons blood work piles into December — an FSA balance that expires, a deductible already met, and a January physical you'd rather walk into with numbers in hand. Here is the list, priced, with the fasting flags and turnaround times that decide the order-by date.

Quick Answer
Start with the four tests a physical orders — CMP, CBC with differential, lipid panel and TSH — which the General Wellness Panel bundles for $49.99 plus one $6 fee, drawn fasting at any Quest or Labcorp with results in 1–3 business days. Add what your history suggests: hemoglobin A1c ($21.99) for blood sugar, vitamin D ($41.99) before winter, ferritin ($24.99) for fatigue, hs-CRP, ApoB and Lp(a) if heart disease runs in the family (the Heart Health Panel bundles them with lipids and A1c for $99.99), and a TB blood test or titers ($91.99) if school or work needs them in January. The expense date for your FSA is the day you are drawn, so order by about December 26 and be drawn by December 30; results can post in January without affecting eligibility. Most tests return in 1–3 business days, A1c and vitamin markers in 2–4, Lp(a) and TB in 3–5 — fast enough to bring the report to a January appointment if you draw before Christmas.

The One-Paragraph Answer
December blood work has three deadlines behind it, and the same list satisfies all of them. An FSA balance expires at the end of the plan year and counts a lab test on the day you are drawn (the FSA deadline guide). A met deductible makes clinician-ordered labs cheap until it resets on January 1 (the deductible guide). And a January physical is a better appointment when the results are already in your hands. The list itself is not complicated: the four tests every physical orders — a comprehensive metabolic panel, a CBC with differential, a lipid panel and a TSH, bundled as the General Wellness Panel for $49.99 — plus the two or three add-ons your history points to. Order by about December 26, be drawn fasting in the morning by December 30, and the results post within a week with the reviewed reference ranges, in time to print for the appointment.
The Checklist, Priced
TestWell prices; one $6 lab processing fee per order covers the physician authorization however many tests you add. "Fasting" means 8–12 hours with water; schedule the draw for the morning and everything on the list can share one visit.
| Tier | Test | Why it's on the list | Price | Fasting | Results |
|---|---|---|---|---|---|
| Everyone — the baseline | Comprehensive metabolic panel | Kidney, liver, glucose, electrolytes — 14 markers | $22.99 | Yes | 1–3 business days |
| CBC with differential | Blood counts, anemia, infection and immune picture | $21.99 | No | 1–3 business days | |
| Lipid panel | Total, LDL, HDL cholesterol and triglycerides | $24.99 | Yes | 1–3 business days | |
| TSH | Thyroid screen | $26.99 | No | 1–3 business days | |
| General Wellness Panel — all four above | The same four tests, one price | $49.99 | Yes | 1–3 business days | |
| Add by history | Hemoglobin A1c | Three-month blood sugar average — family history of diabetes, weight, fatigue | $21.99 | No | 2–4 business days |
| Vitamin D, 25-hydroxy | The most common deficiency to check before winter | $41.99 | No | 1–3 business days | |
| Ferritin | Iron stores — unexplained fatigue, heavy periods, plant-based diets | $24.99 | No | 1–3 business days | |
| Vitamin B12 | Metformin, acid reducers, vegan or vegetarian diets, numbness or tingling | $25.99 | No | 2–4 business days | |
| Heart disease in the family | Heart Health Panel | Lipid panel + ApoB + Lp(a) + hs-CRP + A1c in one draw | $99.99 | Yes | 3–5 business days |
| Lp(a) alone | Inherited risk marker, measured once in most adults | $34.99 | No | 3–5 business days | |
| Instead of building a list | Essential Health Panel | The baseline plus A1c, vitamin D, iron, hs-CRP and fasting insulin — 10 tests | $189.99 | Yes | 2–4 business days |
| Due in January | TB blood test (QuantiFERON) | School, clinical programs, new jobs — one visit, no return trip | $91.99 | No | 3–5 business days |
| Immunity titers | Proof of immunity for school or work when records are missing | From $31.99 | No | 2–5 business days | |
Two ready-made versions of the same idea: the Women's Health Panel ($129.99) adds ferritin, iron, vitamin D and A1c to the core four with a TSH + free T4 thyroid pair; the Men's Health Panel ($139.99) adds testosterone (free and total), PSA, vitamin D and A1c.
The Order-By Calendar
Work backwards from two dates: the last day you can be drawn (December 31, or earlier if your draw site closes), and the day you want results in hand.
- Order: any day — the physician authorization usually arrives within minutes and the requisition is good for weeks. Ordering by December 26 leaves room for a missed morning.
- Draw: by December 30, in the morning, fasting if anything on your list needs it. Most Quest and Labcorp patient service centers take walk-ins, but they close on Christmas Day and New Year's Day and many run shortened hours in between; the lab finder and city pages list each site's schedule. In New York and New Jersey the draw is at BioReference.
- Results: 1–3 business days for the baseline panel, 2–4 for A1c and B12, 3–5 for Lp(a), the Heart Health Panel and the TB test. A December 22 draw returns everything before the new year; a December 30 draw returns most of it in the first week of January — which is fine for your FSA, since the expense date is the draw.
- Physical: download the report from your account or print it; every result comes with the reviewed reference range and a note on what can move it, so the appointment starts from the numbers rather than ending with an order for them.
Which Deadline Is Driving You
- An FSA balance: the draw date is what counts, every test on the list is eligible, and the itemized receipt in your account is the documentation. Match the list to the balance — the FSA deadline guide sorts it by $50, $50–$200 and $200+.
- A met deductible: ask your clinician to order the monitoring tests they want repeated (they are covered at coinsurance until January 1) and buy the rest directly; the deductible guide explains the split.
- An unmet deductible: skip insurance for the routine list entirely — the direct prices above are a fraction of the negotiated rates you would pay toward a deductible that is about to reset anyway.
- Just the physical: the General Wellness Panel is the four tests the office would order; bringing it means the visit can go straight to what the numbers mean.
What to Skip
A deadline is a reason to stop postponing the tests you'd want anyway, not a reason to over-test. Hormone panels without a symptom or a question behind them, food-sensitivity and heavy-metal panels bought on curiosity, and repeating a test that was normal six months ago all spend a balance without changing a decision. The annual blood work guide covers what an annual baseline should and shouldn't include, and the results explainer reads any report you already have against the reviewed ranges before you decide what to add.
Frequently Asked Questions
What blood work should I get before the end of the year?
The four tests a physical orders — comprehensive metabolic panel, CBC with differential, lipid panel and TSH (the General Wellness Panel, $49.99) — plus the add-ons your history suggests: hemoglobin A1c, vitamin D, ferritin or B12, and the Heart Health Panel if heart disease runs in the family. Add a TB blood test or titers if school or work needs them in January.
How late can I order and still have the draw count for this year's FSA?
The FSA expense date is the day you are drawn, so the draw has to happen by December 31 — order by about December 26 and be drawn by December 30 to allow for holiday hours. Results can post in January without affecting eligibility.
Do I need to fast for end-of-year blood work?
For the comprehensive metabolic panel, the lipid panel and anything with glucose or insulin, yes — 8–12 hours with water only. CBC, TSH, A1c, vitamin D, ferritin, B12 and the TB test do not need fasting. Schedule one morning draw and everything on the list can share it.
How long do results take?
1–3 business days for the CMP, CBC, lipid panel, TSH, vitamin D and ferritin; 2–4 for hemoglobin A1c and B12; 3–5 for Lp(a), the Heart Health Panel and the TB blood test. A draw before Christmas returns everything before the new year.
Should I use insurance or pay directly for year-end blood work?
If your clinician will order the tests and you have met your deductible, insurance — monitoring labs are covered at coinsurance until the reset. Otherwise order directly: the routine tests are $21.99–$41.99 each, the receipt is HSA- and FSA-eligible, and you are drawn at the same Quest or Labcorp.
Can I bring TestWell results to my January physical?
Yes. Every result comes as a lab report from Quest, Labcorp or BioReference with the reviewed reference range; download or print it from your account, or share it with your clinician directly. Most physicians accept a recent outside report in place of re-ordering the same tests.
Ready to order your tests?
