TSH receptor antibodies (TRAb, also reported as TBII) are the autoantibodies behind Graves' disease. This binding assay detects any antibody that attaches to the TSH receptor, whether it stimulates the thyroid (as in Graves') or blocks it (an uncommon cause of hypothyroidism), so a positive result in a patient with hyperthyroidism points strongly to Graves' disease rather than a toxic nodule, thyroiditis or excess thyroid medication. It is used at diagnosis — often instead of a radioactive uptake scan — to monitor response to antithyroid drugs, to estimate the chance of relapse when treatment is stopped, and in pregnancy to assess the risk to the baby. The companion TSI test measures the stimulating antibodies specifically; endocrinologists frequently order both. Not available through BioReference, so it cannot be collected in New York or New Jersey.
