Phosphatidylethanol (PEth) is a phospholipid that forms in red blood cell membranes only in the presence of alcohol. Unlike EtG or liver enzyme tests, PEth provides a highly specific and sensitive marker for alcohol consumption over the preceding 3 to 4 weeks, making it virtually impossible to produce a false positive. It is considered the gold standard for monitoring chronic alcohol use in treatment programs, legal proceedings, and clinical care.
Drug and alcohol screening detects recent or chronic substance use for clinical, occupational, or personal monitoring purposes.
Patients being evaluated for substance use, employers requiring confirmation testing, and individuals self-monitoring recovery.

A PEth Alcohol Biomarker test costs $186.99 at TestWell — $192.99 all-in when ordered alone, because the $6 lab processing fee is charged once per order, with no draw fee or physician fee added. Ordered directly from the labs' own consumer sites, the same test lists at $142.00 at Quest Health (list prices checked Sep 12, 2026). Hospital outpatient labs typically bill around $550.00 self-pay for the same test, and often more. With insurance, what you pay depends on your deductible and your plan's negotiated rate; TestWell's price is fixed before the draw and is HSA/FSA-eligible.
