What does a low Calcium mean?

Short answer

Low total calcium is often a false low from low albumin (calcium travels bound to albumin), so the first step is correcting for albumin or measuring ionized calcium. True hypocalcemia comes from low vitamin D, low PTH (after thyroid or parathyroid surgery, autoimmune), kidney disease, low magnesium, and some medications (bisphosphonates, denosumab, loop diuretics). It causes tingling, cramps and muscle spasms.

When does this result need same-day care?

  • Low calcium with muscle spasms, seizures or palpitations — same-day care

What is the normal range for Calcium?

  • age 18+: 8.60 – 10.3 mg/dL

Can reflect vitamin D deficiency, kidney disease, or low albumin (which carries calcium).

What are the common causes of low Calcium?

  1. Low albumin (apparent, not true, low calcium)
  2. Vitamin D deficiency
  3. Hypoparathyroidism: after neck surgery, autoimmune, genetic
  4. Chronic kidney disease
  5. Low magnesium
  6. Medications: bisphosphonates, denosumab, loop diuretics, some anticonvulsants
  7. Acute pancreatitis, large transfusions

What symptoms go with low Calcium?

  • Tingling around the mouth and fingertips
  • Muscle cramps and spasms
  • Fatigue, anxiety
  • Seizures or heart rhythm changes when severe

What do clinicians usually check next?

  • Albumin-corrected or ionized calcium
  • PTH, 25-hydroxyvitamin D, magnesium, phosphorus, kidney function
  • Medication review

Common questions

My calcium is slightly low but my albumin is low too. Is that real?

Probably not. Roughly 0.8 mg/dL of calcium is added for every 1 g/dL the albumin is below 4; ionized calcium settles it.
