Does creatine affect blood test results like creatinine and eGFR?
Short answer
Creatine supplements raise serum creatinine — creatine breaks down into creatinine — so a creatinine-based eGFR reads lower than the kidneys' true filtration, usually by a modest amount at a 3–5 g daily dose. It is a measurement artifact, not kidney damage; cystatin C is unaffected and is the clean alternative. Pause creatine 1–2 weeks before a kidney-function test, or tell the clinician you take it.

Why does creatinine rise without kidney harm?
Serum creatinine is used to estimate kidney filtration on the assumption that the body produces it at a steady rate from muscle. Creatine supplementation increases that production, so creatinine rises even though the kidneys are filtering normally. Studies in healthy adults at recommended doses have not shown kidney injury; the number moves because the input changed, not the filter.
The same logic applies to people with a lot of muscle mass and to heavy meat eaters. Cystatin C is produced by all cells at a rate independent of muscle and diet, which is why it is the better kidney marker in those groups.
How long before a test should you stop creatine?
A single dose raises creatinine for about a day; steady supplementation keeps it elevated. Allow 2–4 weeks off creatine for creatinine to return to your baseline. If stopping is not practical — many people take it year-round — ask for cystatin C alongside creatinine so the eGFR can be calculated both ways.
Common questions
Should I stop taking creatine before a blood test?
If the test is specifically for kidney function, stopping for 1–2 weeks gives a cleaner creatinine. For a general panel it's enough to mention it and ask for cystatin C if eGFR reads low.
Is creatine bad for your kidneys?
At standard doses in people with healthy kidneys, current evidence says no. If you have kidney disease or take medications that affect the kidneys, discuss it with your clinician before starting.
My eGFR is 78 and I take creatine. Do I have kidney disease?
Not necessarily. eGFR 60–89 with no protein in the urine and a normal cystatin C is not kidney disease by definition — it's a creatinine artifact until proven otherwise.
