D-Dimer is a small protein fragment produced when a blood clot breaks down through fibrinolysis. Elevated D-Dimer levels indicate that abnormal clotting and clot breakdown are happening somewhere in the body. The test is most often used to rule out venous thromboembolism (VTE) — including deep vein thrombosis (DVT) and pulmonary embolism (PE) — in patients with a low clinical pretest probability. A negative D-Dimer in a low-risk patient effectively excludes VTE without further imaging. D-Dimer is also used to monitor disseminated intravascular coagulation (DIC), assess recurrent VTE risk after stopping anticoagulation, and evaluate ongoing fibrinolysis. Note: D-Dimer is sensitive but not specific — it can be elevated in pregnancy, post-surgery, infection, malignancy, and aging, so an elevated result requires clinical context.
