What does a high Total Testosterone mean?
Short answer
In men, a high total testosterone is most often from testosterone therapy or anabolic steroid use; rarer causes include adrenal or testicular tumors, and a very high SHBG can raise total while free testosterone is normal. In women, elevated testosterone commonly reflects PCOS; a marked elevation raises concern for an adrenal or ovarian tumor and needs prompt evaluation.

Common questions
Is high testosterone good for men?
Within the normal range there's no evidence higher is better. Above-range levels from therapy raise hematocrit and can worsen sleep apnea and fertility, which is why treated men are monitored.
