What does a low Haptoglobin mean?
Short answer
Haptoglobin binds free hemoglobin released when red cells break down, and the complex is cleared quickly — so a low or undetectable haptoglobin is one of the most sensitive signs of hemolysis. Autoimmune hemolytic anemia, sickle cell disease, mechanical heart valves, G6PD deficiency, transfusion reactions and drug-induced hemolysis all lower it. Liver disease and a normal inherited variant can lower it without hemolysis.

What are the common causes of low Haptoglobin?
- Hemolytic anemia: autoimmune, drug-induced, G6PD deficiency, hereditary spherocytosis, sickle cell, thalassemia
- Mechanical heart valves, march hemoglobinuria (long-distance running), microangiopathic conditions
- Transfusion reaction, malaria
- Liver disease (reduced production)
- Congenital ahaptoglobinemia (inherited, harmless, more common in people of African or East Asian ancestry)
- Newborns (normal)
What symptoms go with low Haptoglobin?
- Fatigue, pallor, breathlessness
- Yellow eyes, dark urine, enlarged spleen
- Often none when the cause is inherited low production

Common questions
What does undetectable haptoglobin mean?
Either active hemolysis — check the reticulocyte count, LDH and bilirubin — or an inherited inability to make haptoglobin, which is harmless. A normal CBC and LDH point to the latter.
