What does a low Free PSA % mean?
Short answer
Free PSA is the fraction of PSA not bound to proteins. When total PSA is in the gray zone (about 4–10 ng/mL), a low free-PSA percentage — under roughly 25%, and especially under 10% — raises the likelihood that the elevation comes from prostate cancer rather than benign enlargement, because cancer cells release proportionally more bound PSA. It refines the biopsy decision; it does not diagnose anything on its own.
