What does a high Free T4 mean?

Short answer

High Free T4 with a low TSH indicates hyperthyroidism — the thyroid is making too much hormone, or the levothyroxine dose is too high. High Free T4 with a normal or high TSH is unusual and points to assay interference (biotin), a sample drawn hours after a dose, or rare pituitary causes. Timing of the draw matters.

What are the common causes of high Free T4?

  1. Graves' disease, toxic thyroid nodules or thyroiditis
  2. Levothyroxine over-replacement, or a draw within a few hours of a dose
  3. Biotin supplement interference
  4. Rarely, a TSH-secreting pituitary adenoma or thyroid hormone resistance
  5. Some medications (amiodarone, heparin) and acute illness
