What does a high Lipase mean?
Short answer
Lipase is the pancreas enzyme, and a level more than three times the upper limit with abdominal pain is diagnostic of acute pancreatitis. Milder elevations are common and less specific: kidney disease (lipase is cleared by the kidneys), gallstones, bowel obstruction or inflammation, some medications, alcohol, and diabetes medications like GLP-1 agonists. Persistently mild high lipase in a well person is usually benign.

What are the common causes of high Lipase?
- Acute pancreatitis (gallstones and alcohol are the leading causes)
- Chronic kidney disease (reduced clearance)
- Gallbladder disease, bowel obstruction, perforation, celiac disease, inflammatory bowel disease
- Medications: GLP-1 agonists (semaglutide, tirzepatide), opioids, some diuretics, valproate, azathioprine
- Alcohol use, high triglycerides, diabetic ketoacidosis
- Pancreatic cyst or tumor (less common)
- Macrolipase (benign assay artifact)
What symptoms go with high Lipase?
- Severe upper-abdominal pain radiating to the back, nausea, vomiting (pancreatitis)
- Often none with mild elevations

- No pain: repeat fasting in a few weeks; kidney function, triglycerides, calcium, liver panel

Common questions
I take semaglutide and my lipase is high. Should I stop?
GLP-1 agonists commonly raise lipase modestly without pancreatitis. Don't stop on your own; tell the prescriber, and treat new persistent abdominal pain as urgent.
Can lipase be high without pancreatitis?
Yes, frequently — kidney impairment, bowel conditions, medications and a benign form called macrolipase all do it. Pancreatitis usually means a large rise plus pain.
