## What does the PCOS Panel cover?

Free and total testosterone, DHEA-S, LH, FSH, prolactin, SHBG, fasting insulin, hemoglobin A1c and TSH — the blood work for diagnosing PCOS and ruling out the conditions that mimic it.

Polycystic ovary syndrome is diagnosed clinically — two of three findings: irregular or absent periods, signs of excess androgens, and polycystic ovaries on ultrasound — but blood work is what confirms the androgen excess and excludes look-alikes. ## How do you prepare?

Fast 9–12 hours (water is fine) and draw in the morning. If you have periods, day 3 of your cycle is the standard timing for LH, FSH and estradiol-axis hormones; if cycles are irregular or absent, any day is fine. Hormonal contraception suppresses several of these values — tell your clinician.


## Common questions about the PCOS Panel

### What blood tests are done for PCOS?

Androgens first — total and free testosterone, with SHBG and DHEA-S — because elevated androgens are one of the three diagnostic criteria. Then the tests that rule out conditions that mimic PCOS: TSH for thyroid disease, prolactin for a pituitary cause, and often 17-hydroxyprogesterone for non-classic adrenal hyperplasia (ordered separately when DHEA-S is high). LH and FSH support the picture, and fasting insulin with A1c assess the insulin resistance that drives most cases. This panel covers all but the 17-OHP.

### What hormone levels indicate PCOS?

There is no single cutoff. A total testosterone in the upper range or above (commonly over about 45–60 ng/dL), a raised free testosterone, a low SHBG, an LH-to-FSH ratio above 2, and a raised DHEA-S in about a quarter of women are the typical pattern, but many women with PCOS have normal androgens on a single draw. That is why the diagnosis rests on symptoms and ultrasound together with the blood work, and why the results should be read by a clinician.
