## What does the Fatty Liver Panel cover?

Liver function panel, GGT, lipid panel, hemoglobin A1c, ferritin and a CBC: the blood work for evaluating and tracking fatty liver disease (MASLD/NAFLD), with every input needed to calculate a FIB-4 fibrosis score.

Fatty liver disease affects about one in three US adults and is usually silent — the first clue is often a mildly raised ALT on a routine panel. Evaluating it well means looking at the liver and at what is driving the fat: this panel combines the seven-marker liver function panel (ALT, AST, ALP, bilirubin, albumin, total protein) with GGT (the enzyme most sensitive to alcohol and bile-duct stress), a lipid panel and hemoglobin A1c for the metabolic syndrome that causes most fatty liver, ferritin (often elevated in fatty liver and a screen for iron overload, which damages the liver on its own) and a CBC for the platelet count. With AST, ALT, platelets and your age, you can calculate the FIB-4 score that clinicians use to decide whether a liver stiffness scan or a hepatology referral is needed. It is the right panel if you have been told you have fatty liver on an ultrasound, have an unexplained ALT elevation, carry weight around the middle, have type 2 diabetes or prediabetes, or drink regularly. Fast 9–12 hours for the lipid and glucose-related values.

## Who should order it?

Adults with fatty liver on imaging, an unexplained ALT or AST elevation, abdominal obesity, type 2 diabetes or prediabetes, high triglycerides, or regular alcohol use who want to evaluate and track liver health.

Fast 9–12 hours (water is fine). No alcohol for 24 hours and no strenuous exercise for 24–48 hours before the draw — both can raise liver enzymes.

## Common questions about the Fatty Liver Panel

### What blood tests show fatty liver disease?

No blood test images fat in the liver — ultrasound does that — but blood work shows its effects and its causes. ALT is typically mildly raised (often under twice the upper limit), with AST lower than ALT in metabolic fatty liver; GGT rises with alcohol and bile-duct stress; triglycerides, HDL and A1c show the metabolic syndrome behind it; and ferritin is often elevated. The FIB-4 score, calculated from age, AST, ALT and platelet count, estimates the chance of significant scarring: below 1.3 is low risk, above 2.67 warrants a stiffness scan or specialist referral.

### How often should fatty liver blood tests be repeated?

Every 6 to 12 months while you are changing diet, activity or weight, since ALT and triglycerides respond within weeks to months and falling numbers confirm the plan is working. Annually once stable. A rising FIB-4, a platelet count that starts to fall, or an AST that overtakes ALT are the signals to see a clinician sooner.
