## What does the COVID-19 Antibody Test test tell you?

Detects antibodies from COVID-19 infection or vaccination to assess immune response.

This test detects IgG antibodies to SARS-CoV-2, indicating a prior infection or immune response to vaccination. It measures antibodies to the spike protein, which are produced by both infection and most vaccines. While the presence of antibodies suggests some level of immunity, the exact level of protection they confer is still being studied.

**Clinical use:** Detects IgG antibodies to SARS-CoV-2. Distinguishes past infection from vaccine-only immunity using N-protein (infection-only) vs S-protein (both) antibodies. Useful for assessing prior exposure or vaccine response in immunocompromised patients.

## Who should order it?

Anyone curious about past asymptomatic COVID infection, immunocompromised patients evaluating vaccine response, post-COVID convalescent plasma donors, anyone investigating Long COVID symptoms.

## Biomarkers measured

- SARS-CoV-2 Spike IgG

## How much does a COVID-19 Antibody Test test cost?

A COVID-19 Antibody Test test costs $51.99 at TestWell — $57.99 all-in when ordered alone, because the $6 lab processing fee is charged once per order, with no draw fee or physician fee added. Hospital outpatient labs typically bill around $180.00 self-pay for the same test, and often more. With insurance, what you pay depends on your deductible and your plan's negotiated rate; TestWell's price is fixed before the draw and is HSA/FSA-eligible.

## Common questions about the COVID-19 Antibody Test test

### What does the COVID antibody test detect?

IgG antibodies to SARS-CoV-2. Most tests measure antibodies to the spike (S) protein, which both natural infection AND vaccines produce. Some tests also measure nucleocapsid (N) protein antibodies, which only develop after natural infection — useful for distinguishing past illness from vaccine-only immunity.

### Does a positive COVID antibody test mean I'm immune?

Partially. Antibodies indicate prior exposure (natural or vaccine), and higher levels generally correlate with reduced severity of subsequent infection. But there's no validated cutoff that guarantees immunity, and antibody levels wane over months. Cellular immunity (T-cell memory) provides additional protection that this test doesn't measure.

### How long after infection do antibodies appear?

IgG antibodies typically develop 2-3 weeks after symptom onset and peak around 4-6 weeks. They can persist for 6-12 months or longer, often gradually declining. Test at least 3-4 weeks after suspected infection or vaccination for reliable results.

### How much does a COVID antibody test cost without insurance?

On TestWell, the COVID Antibody Test is $51.99. Quest Direct lists the same test at $69, walk-in hospital labs typically bill $130-180 without insurance.

### What is the CPT code for COVID antibody testing?

The CPT code for SARS-CoV-2 antibody testing is 86769 (qualitative or semi-quantitative IgG). Spike-specific IgG is sometimes billed as 86328.

### Do I need to fast?

No fasting required. Any time of day.

### Should I order antibody testing vs an active COVID test?

Different uses. Antibody testing detects PAST exposure (infection or vaccine). For active infection, use a PCR or rapid antigen swab test (we don't currently offer these). Antibody testing is most useful for retrospective questions ('did I have COVID last winter?') and for immune-response assessment in immunocompromised patients.
