# Does lack of sleep affect blood test results?

> One bad night moves the stress and metabolic markers: cortisol is higher and its morning peak shifts, glucose and insulin resistance rise, testosterone falls (by roughly 10–15% after a week of five-hour nights in young men), and CRP, appetite hormones and TSH can drift. Night-shift workers have inverted cortisol rhythms, so a 'morning' cortisol drawn after a shift is not comparable to the reference range. Test after a normal night when you can, and tell the clinician about shift work.

## Why does sleep matter for hormone panels?

Testosterone is made mostly during sleep, so a man tested after a run of short nights can land below the reference range and be labeled hypogonadal when he is sleep-deprived. Endocrine guidelines already require two separate morning measurements before diagnosis; adding 'after normal sleep' to both is the practical safeguard.

Cortisol follows a circadian rhythm that is anchored to wake time, not clock time. Shift workers who sleep by day have their peak in the evening; a draw at 8 a.m. after a night shift may read low and prompt an adrenal work-up that is not needed.

## How do short nights affect glucose?

Even a few nights of four-to-five-hour sleep reduce insulin sensitivity measurably in healthy adults, lifting fasting glucose and insulin. A1c is unaffected by a single week, which is one reason it is the steadier screening test; a fasting glucose that lands just over a threshold after poor sleep deserves a repeat rather than a diagnosis.

### Should I reschedule my blood test if I didn't sleep?

For routine panels, no — the effect is small. For cortisol, testosterone, fasting glucose or insulin, testing after a normal night gives a more reliable result and can spare you a repeat.

### I work nights. When should I do a cortisol test?

Within an hour or two of your usual waking time, and write your sleep schedule on the requisition so the result is read against your rhythm, not the clock.
