# What should you avoid before a PSA blood test — and what doesn't matter?

> A PSA test needs no fasting, and coffee, alcohol and food do not meaningfully change it. What does move PSA in the short term is anything that irritates or presses on the prostate in the day or two before the draw: ejaculation, a long bike ride, a digital rectal exam, a recent catheter, biopsy or cystoscopy, and — more than any of those — a urinary tract infection or prostatitis, which can keep PSA elevated for weeks after treatment. One class of medicines lowers it: finasteride and dutasteride (5-alpha-reductase inhibitors) roughly halve PSA after months of use, so the person reading the result needs to know you take one; do not stop it for the test. Testosterone therapy can raise PSA and is monitored for that reason. The practical rules: abstain from ejaculation and skip hard cycling for about two days, have the blood drawn before any rectal exam, and wait until a urinary infection is fully treated.

## Which results does it affect?

| Marker | Direction | How long the effect lasts | What to do | Source |
| --- | --- | --- | --- | --- |
| PSA (total) | can raise | Ejaculation within about the previous two days | Abstain for 48 hours before the draw | [American Cancer Society — Screening tests for prostate cancer (factors that affect PSA: ejaculation, cycling, prostatitis, DRE, 5-ARIs)](https://www.cancer.org/cancer/types/prostate-cancer/detection-diagnosis-staging/tests.html) |
| PSA (total) | can raise | Vigorous cycling or other prolonged pressure on the perineum shortly before | Skip long rides for two days | [American Cancer Society — Screening tests for prostate cancer (factors that affect PSA: ejaculation, cycling, prostatitis, DRE, 5-ARIs)](https://www.cancer.org/cancer/types/prostate-cancer/detection-diagnosis-staging/tests.html) |
| PSA (total) | can raise | After a digital rectal exam, catheter, cystoscopy or prostate biopsy — days to weeks depending on the procedure | Draw PSA before a rectal exam; after a procedure, ask how long to wait | [National Cancer Institute — Prostate-Specific Antigen (PSA) Test fact sheet](https://www.cancer.gov/types/prostate/psa-fact-sheet) |
| PSA (total) | raises | Prostatitis or a urinary tract infection; can stay high for weeks after treatment | Treat the infection first, then retest | [American Cancer Society — Screening tests for prostate cancer (factors that affect PSA: ejaculation, cycling, prostatitis, DRE, 5-ARIs)](https://www.cancer.org/cancer/types/prostate-cancer/detection-diagnosis-staging/tests.html) |
| PSA (total) | falsely lowers | Finasteride or dutasteride lower PSA by about half after months of use | Keep taking it; tell whoever interprets the result | [National Cancer Institute — Prostate-Specific Antigen (PSA) Test fact sheet](https://www.cancer.gov/types/prostate/psa-fact-sheet) |
| Free PSA and % free PSA | raises or lowers | Moved by the same factors as total PSA | Same preparation; draw both from the same sample | [Testing.com (AACC) — PSA](https://www.testing.com/tests/prostate-specific-antigen-psa/) |
| Food, fasting, coffee, alcohol | interferes | No meaningful effect | No fasting required — eat and drink normally | [Testing.com (AACC) — PSA](https://www.testing.com/tests/prostate-specific-antigen-psa/) |

## What moves a PSA level in the short term

PSA is a protein the prostate leaks into the blood, and the amount leaking rises when the gland is stimulated, inflamed or handled. Ejaculation raises it for a day or two, which is why most laboratories and urology practices ask for about 48 hours of abstinence before the draw. Long or hard cycling, with its sustained pressure on the perineum, can do the same. A digital rectal exam has a smaller effect but the sensible order is blood first, exam second; a biopsy, cystoscopy or catheter is a larger disturbance and the wait before a meaningful PSA is longer — ask the clinician who did the procedure. The biggest short-term distortion is infection: prostatitis or a urinary tract infection can lift PSA well into the range that prompts a biopsy and keep it there for weeks after the antibiotics are finished, so a PSA drawn during or just after an infection is not a screening result at all.

## What does not matter — alcohol, coffee, food and the time of day

PSA is not a fasting test. Eating, coffee and alcohol the night before do not change the result in any clinically meaningful way, and there is no strong morning-versus-afternoon pattern to plan around. If a PSA is being drawn with a testosterone level or a fasting panel, follow the rules for those tests — the PSA will not be hurt by them.

## Medicines and therapies that shift PSA

Finasteride (Propecia, Proscar) and dutasteride (Avodart) shrink the prostate and roughly halve PSA after six to twelve months; clinicians double the measured value to compare it against the usual thresholds, so the result is only interpretable if they know you take one. Do not stop the medicine for the test. Testosterone therapy can raise PSA, which is why guidelines check PSA before starting and during treatment; a rise on TRT is a reason to talk to the prescriber, not to stop the test.

## If your PSA came back high

A single elevated PSA is a reason to repeat the test, not to panic — guidelines recommend confirming a mildly elevated value a few weeks later before any biopsy decision, and the repeat should be drawn with the triggers above out of the way: no ejaculation or cycling for two days, no recent rectal exam or procedure, no active urinary symptoms. Age, prostate size and benign enlargement all raise PSA without cancer; the trend over time and the free-to-total ratio carry more information than one number.

## PSA tests you can order yourself

A PSA screen or a repeat after a high result does not need a referral. Total PSA is the screening test; Free & Total PSA adds the ratio that helps separate benign enlargement from a result worth pursuing. Prepare as above — two days without ejaculation or hard cycling, no active urinary infection — and the result is interpretable.

Prices are per test; one $6 lab processing fee is added per order.

## Common questions

### Can I drink alcohol the night before a PSA test?

Alcohol does not meaningfully change a PSA result. If the PSA is being drawn alongside a liver panel, triglycerides or testosterone, follow the alcohol rules for those tests instead.

### How long should I abstain from ejaculation before a PSA test?

About 48 hours. Ejaculation can raise PSA for a day or two; most laboratories and urologists ask for two days of abstinence before the draw.

### Do I need to fast for a PSA blood test?

No. PSA is not affected by food or drink. Fast only if other tests on the same order require it.

### Does riding a bike affect a PSA test?

A long or hard ride shortly before the draw can raise PSA temporarily through pressure on the perineum. Skip cycling for about two days before the test.

### Does finasteride or dutasteride affect PSA?

Yes — they lower PSA by about half after months of use. Keep taking the medicine and make sure whoever reads the result knows you are on it; the measured value is usually doubled for interpretation.
