# Should you take levothyroxine before a thyroid blood test?

## Why the dose timing matters for T4 but not TSH

TSH is the pituitary's slow-moving response to weeks of thyroid hormone exposure, so it doesn't jump after one tablet — which makes it the marker clinicians use to adjust doses. Free T4, on the other hand, rises measurably for a few hours after each dose. A sample drawn two hours post-pill can read high and prompt a needless dose cut.

After a dose change, wait 6–8 weeks before rechecking TSH; it takes that long to reach a new steady state. Testing sooner shows a moving target.

## The practical routine

Book a morning draw, bring the tablet with you, and take it as soon as the draw is done with water on an empty stomach (levothyroxine absorbs best 30–60 minutes before food, coffee, calcium or iron). Never stop the medication to 'see what your levels really are' — that is a clinical decision, and stopping distorts the result anyway.

## Common questions

### I already took my levothyroxine this morning. Should I still test?

TSH will be fine. If Free T4 is on the order, either wait to draw at least 4–6 hours after the dose or reschedule for tomorrow morning pre-dose. Note the timing for whoever interprets the result.

### How soon after starting or changing a dose should I retest?

6–8 weeks. That's how long TSH needs to settle.
