# Should you stop iron supplements before an iron blood test?

> Hold your iron pill on the morning of the draw and test fasting: a supplement taken hours earlier can push serum iron and transferrin saturation well above your true baseline. Ferritin — the iron-stores marker — is barely affected by a single dose and can be drawn any time, though acute illness raises it.

## Which results does it affect?

| Marker | Direction | How long the effect lasts | What to do | Source |
| --- | --- | --- | --- | --- |
| Iron (serum) | raises | Hours after a dose (peaks ~2–4 h) | Skip the dose that morning; draw fasting | [NIH Office of Dietary Supplements — Iron fact sheet](https://ods.od.nih.gov/factsheets/Iron-HealthProfessional/) |
| Transferrin saturation | raises | Hours after a dose | Skip the dose that morning | [NIH Office of Dietary Supplements — Iron fact sheet](https://ods.od.nih.gov/factsheets/Iron-HealthProfessional/) |
| TIBC | interferes | Not meaningfully affected by a single dose | None | [NIH Office of Dietary Supplements — Iron fact sheet](https://ods.od.nih.gov/factsheets/Iron-HealthProfessional/) |
| Ferritin | interferes | Not affected by a single dose; rises with inflammation or infection | Test when well; no need to hold iron | [NIH Office of Dietary Supplements — Iron fact sheet](https://ods.od.nih.gov/factsheets/Iron-HealthProfessional/) |

## Ferritin versus serum iron

Serum iron measures what's circulating right now, so it swings with meals and pills and is at its most stable fasting in the morning. Ferritin reflects stored iron and changes slowly over weeks — the reason it's the marker that tells you whether supplementation is working. Recheck ferritin about 8–12 weeks into supplementation, not after a few days.

Ferritin is also an acute-phase protein: a cold, an infection or any inflammation can raise it temporarily and hide a deficiency. If you were sick in the past two weeks, wait.

## Common questions

### How long should I stop iron before a test?

For serum iron and saturation, just the morning of the draw (skip that one dose). Ferritin doesn't require stopping.

### Does eating a steak the night before change my iron test?

Slightly for serum iron, which is why a fasting morning draw is recommended. Ferritin isn't affected.
