Warfarin raises the PT/INR — that is the point, and the test is how the dose is managed. Heparin prolongs the aPTT. The newer oral anticoagulants (apixaban, rivaroxaban, dabigatran, edoxaban) can prolong PT and aPTT unpredictably, so those tests do not measure how well they are working. Aspirin and clopidogrel do not change PT/INR, aPTT or the platelet count at all — they change how platelets function, which routine tests do not measure. Vitamin K in food lowers the INR on warfarin. Warfarin works by lowering vitamin K-dependent clotting factors, so the prothrombin time lengthens and the INR — a standardized version of it — rises into the range the prescriber chooses. Heparin and its low-molecular-weight relatives act on antithrombin and prolong the aPTT, which is how unfractionated heparin is monitored in hospital. The direct oral anticoagulants block a single factor directly; they can stretch PT or aPTT, but the change does not track the dose, so a normal PT on apixaban does not mean the drug is not working and a long one does not mean too much. Aspirin, clopidogrel, ticagrelor and prasugrel are antiplatelet drugs. They make platelets less sticky without changing how many there are or how fast plasma clots, so the platelet count, PT/INR and aPTT stay normal. That is why a normal platelet count on aspirin is exactly what is expected. ## Preparing for a draw while on a blood thinner Do not stop or skip a dose for a routine blood draw — the risk of a clot outweighs a slightly longer bleed at the puncture site, and the phlebotomist will hold pressure a little longer. If the test is being drawn to check the drug itself — an INR on warfarin, an anti-Xa level on heparin — the prescriber sets the timing relative to the dose, and consistency matters more than the clock time. Tell the lab which anticoagulant you take, because it changes how some results are interpreted, and mention antibiotics, illness or diet changes to the clinic that manages your warfarin, since all three shift the INR. ## Common questions ### Does aspirin affect PT/INR or platelet count? No. Aspirin changes how platelets work, not how many there are or how quickly plasma clots, so PT/INR, aPTT and the platelet count read normal on aspirin. ### Should I stop my blood thinner before a blood test? No — never without the prescriber's instruction. A routine venipuncture is safe on anticoagulants; the phlebotomist simply holds pressure longer. ### Why is my PT/INR abnormal on Eliquis or Xarelto?
