## What is a CBC with differential?

A complete blood count (CBC) is the most frequently ordered blood test in medicine. From one lavender-top tube an automated hematology analyzer counts the three cell lines in your blood — red cells, white cells and platelets — and measures the hemoglobin they carry and the size and shape of the red cells. The **differential** ("diff") is the extra step that sorts the white blood cells into their five types and reports each as a percentage and an absolute count.

You will see the test written several ways on requisitions and reports: "CBC with differential", "CBC w/ diff", "CBC with auto differential", "hemogram with differential" and, at Quest, "CBC (includes Differential and Platelets)". They are the same test. A plain "CBC" or "CBC without differential" reports the counts and red-cell indices but not the white-cell breakdown.

The differential is what turns a white cell count from a single number into a pattern. A high white count with high neutrophils tells a different story from a high white count with high lymphocytes, and a low count of one type can matter even when the total is normal. That is why most clinicians order the version with the differential unless they are only tracking hemoglobin or platelets.

## What does “auto differential” mean on my report?

"Auto" (automated) differential means the analyzer classified the white cells itself, using light scatter, electrical impedance and staining to tell the five types apart. Modern analyzers classify tens of thousands of cells per sample in seconds, which makes the automated differential more precise than a human counting 100 cells on a slide — for normal and near-normal blood.

When the analyzer meets something it cannot classify confidently — immature or abnormal-looking cells, very high or very low counts, clumped platelets, nucleated red cells — it raises a flag and the lab performs a **manual differential**: a technologist spreads a drop of blood on a slide, stains it and counts and describes the cells under a microscope. Your report may then show "manual diff" or a comment such as "smear reviewed", and it can include extra lines like "bands", "metamyelocytes" or "atypical lymphocytes" that an automated count never reports.

Seeing "auto differential" on your report is the normal case and is not a sign of anything wrong. A manual review is also not by itself alarming — reasons range from a simple viral infection to platelet clumping in the tube — but the comment attached to it is worth reading and discussing with whoever ordered the test.

## What do the red blood cell values mean — hemoglobin, hematocrit and the indices?

Red cells carry oxygen, and the red-cell section of the CBC answers two questions: do you have enough of them, and are they the right size and color? Hemoglobin is the oxygen-carrying protein and the single number that defines anemia; hematocrit is the share of your blood volume made of red cells; the red cell count is the number of cells. The three move together, so they are read as a group.

The **indices** describe the average red cell. MCV is its size, MCH and MCHC are how much hemoglobin each cell holds, and RDW is how much the sizes vary. These are the lines that give a low hemoglobin its direction: small pale cells (low MCV, low MCHC) point to iron deficiency or thalassemia; large cells (high MCV) point to vitamin B12 or folate deficiency, alcohol, liver disease, thyroid disease or certain medications; normal-sized cells with a low count point to blood loss, kidney disease, chronic illness or a marrow problem. A high RDW alongside a normal MCV can mean two processes at once, or an early deficiency that has not yet shifted the average.

## What does each white blood cell type in the differential do?

White cells are the immune system in transit, and each of the five types has a job. The differential reports every type two ways: as a **percentage** of all white cells and as an **absolute count** (the percentage multiplied by the total white count, written like "ANC" for absolute neutrophil count). Absolute counts are the more meaningful number — a "high" lymphocyte percentage is often just the arithmetic of a low neutrophil count, not an excess of lymphocytes.

The total white cell count ([WBC](https://www.test-well.com/biomarkers/white-blood-cell-count), reference 4.50 – 11.0 10^3/uL) sits above the differential. It can be normal while one type is clearly abnormal, which is precisely why the differential is ordered.

## What do the platelet count and MPV show?

Platelets are the cell fragments that plug leaks in blood vessels. The [platelet count](https://www.test-well.com/biomarkers/platelet-count) (reference 150 – 450 10^3/uL) falls with certain infections, immune conditions, liver disease, heavy alcohol use, some medications and marrow disorders, and rises with iron deficiency, inflammation, recovery from bleeding or surgery, after spleen removal and — uncommonly — with a bone marrow disorder. Bleeding risk from a low count generally begins well below the reference range.

[MPV](https://www.test-well.com/biomarkers/mpv) (mean platelet volume, reference 7.50 – 11.5 fL) is the average platelet size. Large platelets suggest the marrow is producing them quickly, as happens when they are being destroyed or consumed; small platelets suggest reduced production. A falsely low platelet count from clumping in the tube is a common reason for a manual smear review and a repeat draw.

## How do you read the common CBC patterns?

No single CBC line is diagnostic, but a handful of patterns come up over and over. They are starting points for a conversation with a clinician, not conclusions.

- **High WBC with high neutrophils** — the classic response to a bacterial infection or acute inflammation; also seen with steroids, smoking, intense exercise and stress. Very high counts with immature forms ("left shift", bands) point to a more significant infection or, rarely, a marrow disorder.
- **High lymphocytes** — most often a viral infection, including mononucleosis; persistent, marked elevation in an older adult is one of the ways chronic lymphocytic leukemia is first noticed.
- **Low neutrophils (neutropenia)** — a recent viral illness, certain medications, autoimmune disease, vitamin deficiency or, in people of African or Middle Eastern ancestry, a benign inherited variant. Infection risk rises as the absolute neutrophil count falls below about 1.0 ×10³/µL, and sharply below 0.5.
- **High eosinophils** — allergies, asthma, eczema, a drug reaction or a parasitic infection; a markedly high count is investigated further.
- **Low hemoglobin with low MCV** — iron deficiency until proven otherwise; ferritin and an iron panel come next, and in men and postmenopausal women a search for the source of blood loss.
- **Low hemoglobin with high MCV** — vitamin B12 or folate deficiency, alcohol, liver or thyroid disease, or medications such as methotrexate and some antivirals; B12, folate and often methylmalonic acid follow.
- **High hemoglobin and hematocrit** — dehydration, smoking, living at altitude, sleep apnea or testosterone therapy; persistent unexplained elevation is evaluated for polycythemia vera.
- **All three lines low (pancytopenia)** — needs prompt evaluation for marrow suppression from medications, infection, deficiency or a marrow disorder.

## How do you get a CBC with differential, and what should you expect?

The CBC needs no fasting and no special preparation; it can be drawn at any time of day, alone or alongside a metabolic panel and other tests from the same visit. Strenuous exercise in the hour before a draw can temporarily raise the white count, and dehydration concentrates the blood and lifts hemoglobin and hematocrit, so a well-hydrated, unhurried draw gives the cleanest baseline.
