# What does a low TIBC mean?

> Low TIBC means less transferrin in circulation. It occurs with iron overload (transferrin is down-regulated when iron is plentiful), with inflammation and chronic disease (transferrin is a negative acute-phase protein), malnutrition, liver disease and kidney protein loss. With a high saturation it points toward hemochromatosis; with a high ferritin and low iron, toward inflammation.

1. Iron overload: hereditary hemochromatosis, transfusions, excess supplementation
2. Inflammation, infection, chronic disease

- Usually none from TIBC itself
- Joint pain, fatigue, bronze skin in overload; symptoms of the underlying illness otherwise

- Transferrin saturation and ferritin
- CRP and albumin to gauge inflammation or nutrition
- HFE genetic testing if saturation is persistently above 45%

## Common questions

### Does low TIBC mean I have too much iron?

Only together with a high transferrin saturation and ferritin. Low TIBC with low iron and a normal-to-high ferritin is the inflammation pattern instead.
