# What does a high Prolactin mean?

> Mildly high prolactin is very often medication-related (antipsychotics, metoclopramide, some antidepressants and blood-pressure drugs), stress, a recent meal, exercise, nipple stimulation, pregnancy or breastfeeding, or hypothyroidism. Levels above about 100–200 ng/mL, or a persistent elevation with irregular periods, milky discharge, low libido or headaches, point to a pituitary prolactinoma and are imaged with an MRI.

## What are the common causes of high Prolactin?

1. Medications: antipsychotics (risperidone, haloperidol), metoclopramide, domperidone, verapamil, SSRIs, opioids, estrogens
2. Pregnancy and breastfeeding
3. Stress, exercise, sleep, meals, nipple stimulation, recent venipuncture stress
4. Hypothyroidism
5. Prolactinoma or other pituitary/hypothalamic lesions
6. Kidney or liver failure, chest-wall injury
7. Macroprolactin (a biologically inactive form that some assays count)

## What symptoms go with high Prolactin?

- Irregular or absent periods, infertility
- Milky nipple discharge
- Low libido, erectile dysfunction
- Headaches or vision changes with larger tumors

- Repeat fasting, rested, 2–3 hours after waking, without breast stimulation
- TSH; pregnancy test
- Macroprolactin screen for a moderate elevation without symptoms
- Pituitary MRI for persistent unexplained elevation
