# What does a low MCV mean?

> A low MCV (microcytosis) means small red cells, and in most people it is iron deficiency. The other main cause is thalassemia trait — an inherited, usually harmless condition common in people of Mediterranean, Middle Eastern, South Asian, Southeast Asian and African ancestry — distinguished by a normal ferritin, a normal or high red-cell count and a normal RDW. Lead exposure and chronic disease are less common causes.

- age 18+: 80.0 – 100 fL

Often reflects iron deficiency or thalassemia.

## What are the common causes of low MCV?

1. Iron deficiency
2. Thalassemia trait (alpha or beta)
3. Anemia of chronic disease (sometimes)
4. Lead poisoning, sideroblastic anemia (rare)

## What symptoms go with low MCV?

- Often none
- Fatigue and other anemia symptoms if hemoglobin is low

- Ferritin and iron panel
- Hemoglobin electrophoresis if ferritin is normal
- Look at the RDW and RBC count — high RBC with low MCV suggests thalassemia trait

## Common questions

### Low MCV but normal ferritin — what does that mean?
