# What does a low MCH mean?

> Low MCH means each red cell carries less hemoglobin — the hallmark of iron deficiency and of thalassemia trait, and sometimes of anemia of chronic disease or lead exposure. It travels with a low MCV (small cells). Ferritin separates iron deficiency (low) from thalassemia trait (normal or high), which matters because iron is the wrong treatment for the latter.

## What are the common causes of low MCH?

1. Iron deficiency (blood loss, low intake, malabsorption)
2. Thalassemia trait (alpha or beta)
3. Anemia of chronic disease or inflammation
4. Lead poisoning, sideroblastic anemia (rare)
5. Copper deficiency

## What symptoms go with low MCH?

- Fatigue, breathlessness, pallor if anemic
- Often none in thalassemia trait

## What do clinicians usually check next?

- Ferritin, iron and TIBC, CRP
- Hemoglobin electrophoresis if ferritin is normal and RBC count is high
- Find and treat the source of iron loss

## Common questions

### Low MCH but normal hemoglobin — what does it mean?

Small, pale red cells without anemia is the classic thalassemia-trait picture, or very early iron deficiency. Ferritin and the red cell count tell them apart.
