# Hyaline casts in urine: what they mean, the harmless causes and which casts signal kidney disease

> Hyaline casts are clear protein cylinders formed in the kidney tubules, and a few — up to 2 per low-power field — are normal. More appear with dehydration, exercise, fever, heat and diuretics, when concentrated, slow-flowing urine gels the protein, and they mean nothing in a well person with a normal dipstick. Casts containing red cells, white cells, tubular cells or granular debris are a different matter: each points to a specific kidney disease and needs creatinine, eGFR and a urine albumin-to-creatinine ratio the same week.

## When does this result need same-day care?

- Red-cell casts, or casts with swelling, rising blood pressure and very little urine — acute glomerulonephritis; same-day nephrology care
- Granular or epithelial casts with a rapidly rising creatinine after surgery, sepsis, shock or contrast — acute tubular necrosis; same-day assessment

## What is the normal range for Hyaline Casts?

- age 18+, hyaline; microscopy: 0 – 2 /lpf

Hyaline casts rise with dehydration, exercise, fever and diuretics and are usually harmless; red-cell casts mean glomerular bleeding, white-cell casts kidney infection or inflammation, granular or waxy casts tubular injury or chronic kidney disease — those need prompt evaluation.

## What are the common causes of high Hyaline Casts?

1. Dehydration, heat exposure, fever and emotional stress — concentrated, acidic, slow-flowing urine
2. Strenuous exercise in the last 48 hours — hyaline and sometimes a few granular casts
3. Loop diuretics such as furosemide, and other low-flow states including heart failure
4. Heavy proteinuria of any cause — protein is the raw material for casts; nephrotic syndrome
5. Chronic kidney disease of any cause, where hyaline casts accompany granular, waxy or broad casts
6. Pyelonephritis, where hyaline casts accompany white-cell casts
7. Red-cell casts (not hyaline) — glomerulonephritis and vasculitis; white-cell casts — pyelonephritis and interstitial nephritis; granular and epithelial casts — acute tubular necrosis; fatty casts — nephrotic syndrome; waxy and broad casts — advanced chronic kidney disease

## What symptoms go with high Hyaline Casts?

- None — hyaline casts are found on a routine microscopy, often after a hot day, a long run or a febrile illness
- Thirst, dark urine and light-headedness when dehydration is the cause
- Swelling, foamy urine or rising blood pressure when casts accompany protein and blood
- Reduced urine output and a rising creatinine after surgery, sepsis or contrast when granular casts are present

## What do clinicians usually check next?

- Hyaline casts only with a normal dipstick: hydrate normally and repeat the urinalysis on a rested, well-hydrated day — a normal repeat ends it
- Hyaline casts with protein or blood, or any other cast type: serum creatinine and eGFR, a urine albumin-to-creatinine ratio, blood pressure and a complete blood count
- Red-cell casts or dysmorphic red cells with protein: urgent nephrology referral with ANA, ANCA, complement levels and hepatitis serologies
- White-cell casts: a urine culture and treatment for pyelonephritis, or a review for drug-induced interstitial nephritis (NSAIDs, proton-pump inhibitors, antibiotics) with the drug stopped
- Granular, epithelial or waxy casts: same-day assessment of kidney function when the creatinine is rising, and a nephrology referral for chronic changes
- A kidney ultrasound when obstruction, kidney size or scarring is in question

## Common questions

### What does it mean to have hyaline casts in urine?

In most cases, that the urine was concentrated and flowing slowly through the kidney tubules when the sample formed — after exercise, in hot weather, with a fever, with dehydration or on a diuretic. A few hyaline casts are normal, and more of them in a well person with no protein or blood on the dipstick need only normal hydration and perhaps a repeat. They matter when they come with other cast types, protein or blood.

### How many hyaline casts are normal?

0 to 2 per low-power field, and up to 5 at some laboratories. Any number of red-cell, white-cell, granular, epithelial, fatty or waxy casts is abnormal. An increased hyaline count with an otherwise normal urinalysis and a normal creatinine is not a disease.

### Are hyaline casts a sign of kidney disease?

Not on their own. They are made from a normal kidney protein and increase with dehydration and exertion in healthy kidneys. They do accompany kidney disease — alongside protein, blood and casts of other types — so the surrounding findings decide. Hyaline casts with a negative dipstick and a normal creatinine are physiology; with protein, blood or granular casts they are part of a kidney picture that needs creatinine, eGFR and a urine albumin-to-creatinine ratio.

### Which casts in urine are serious?

Red-cell casts (glomerulonephritis or vasculitis — a nephrology emergency), white-cell casts (kidney infection or interstitial nephritis), granular and epithelial casts with a rising creatinine (acute tubular necrosis), fatty casts (nephrotic syndrome) and broad waxy casts (advanced chronic kidney disease). Hyaline casts are the one type that is normal in small numbers.
