# What does a high Hematocrit mean?

> Hematocrit tracks hemoglobin: a high value most often means dehydration or a stimulus to make more red cells — smoking, low oxygen from lung disease or sleep apnea, altitude, or testosterone therapy. Above about 54% on testosterone therapy prompts a dose change; persistently high values without a cause are evaluated for polycythemia vera.

## What is the normal range for Hematocrit?

- Male, age 18+: 38.8 – 50.0 %
- Female, age 18+: 34.9 – 44.5 %

Often reflects dehydration, lung disease, or living at high altitude.

## What are the common causes of high Hematocrit?

1. Dehydration
2. Smoking, sleep apnea, lung disease, altitude
3. Testosterone or anabolic steroids
4. Polycythemia vera

## What symptoms go with high Hematocrit?

- Often none
- Headache, flushing, itching, dizziness

## What do clinicians usually check next?

- Repeat hydrated
- Oxygen saturation and sleep assessment
- EPO and JAK2 if persistent

## How do you recheck Hematocrit?

## Common questions

### What hematocrit is too high on TRT?

Most guidelines use 54% as the threshold to hold or reduce the dose and consider phlebotomy.
