Infection, autoimmune disease, tissue injury and some cancers raise it; so do anemia, pregnancy, older age and obesity, which is why it is read with CRP and the clinical picture. It rises and falls slowly, over days to weeks. ## When does this result need same-day care? - ESR over ~50 mm/hr in an adult over 50 with new headache, scalp tenderness, jaw pain while chewing or vision changes — same-day care (possible giant cell arteritis) ## What is the normal range for ESR? - Male, age 18–50: 0.000 – 15.0 mm/hr - Male, age 51+: 0.000 – 20.0 mm/hr - Female, age 18–50: 0.000 – 20.0 mm/hr - Female, age 51+: 0.000 – 30.0 mm/hr Often reflects infection, autoimmune disease, or chronic inflammation. Above the age- and sex-adjusted range — roughly over 15–20 mm/hr in men and 20–30 in women. Values over 100 almost always mean a significant infection, inflammatory disease or myeloma and need prompt evaluation. ### Can ESR be high without any disease? Mildly, yes — age, pregnancy, anemia and obesity all raise it.
