# What does a high Direct Bilirubin mean?

> Direct (conjugated) bilirubin is the form the liver has already processed, so a high level means the liver is making it but it isn't draining — bile-duct obstruction from gallstones or a mass, or liver-cell injury from hepatitis, medications or alcohol. It is the fraction that spills into urine and darkens it. High total bilirubin with a normal direct fraction is a different story (Gilbert syndrome, hemolysis).

- Jaundice with fever, severe abdominal pain or confusion — emergency care (possible cholangitis or liver failure)

### Direct vs. indirect bilirubin — which matters?
