# What does a low Calcium mean?

> Low total calcium is often a false low from low albumin (calcium travels bound to albumin), so the first step is correcting for albumin or measuring ionized calcium. True hypocalcemia comes from low vitamin D, low PTH (after thyroid or parathyroid surgery, autoimmune), kidney disease, low magnesium, and some medications (bisphosphonates, denosumab, loop diuretics). It causes tingling, cramps and muscle spasms.

## Common questions

### My calcium is slightly low but my albumin is low too. Is that real?

Probably not. Roughly 0.8 mg/dL of calcium is added for every 1 g/dL the albumin is below 4; ionized calcium settles it.
